UPD: Thanks for the help. Every time I write something in Russian I am plagued with the thought "This is wrong! This is wrong!" ... but I guess it's not always true!

However horrible it sounds to you, it's correct and perfectly understandable.

You can use shorter phrases or reduce it to just:

Пожалуйста, позвоните мне по телефону [N] или напишите на [e-mail].

Other possible phrases are:

(more preferred contacts listed earlier).
2. Со мной можно связаться по телефону +7 (000) 123-4567 или электронной почте someone@example.net

it sounds fine but u missed the 'н' - instead of 'одим' you should use 'одним'.

alternatively, u can say 'Пожалуйста, свяжитесь со мной любым из следующих способов' (ANY of the following methods)- this one is even better i think..
