If you spent a week riding the Vladivostok Express to Moscow in a second-class compartment, with no shower available, you would say that "первым делом в Москве надо принять душ". But if you would say that it was "в первую очередь" you needed to take a shower, that would sound a bit too official/formal.
"первым делом" (="it's main business") - not official speaking style and mostly about you talking sometimes, but may doing often "первым делом - girls" = "girls have the biggest priority for me".
"в первую очередь" (="it's in first row (of task list)" - more official speaking style and mostly about future planning. As priority description means not so global priority, as "первым делом".
