They're somewhat interchangeable, at least in their primary meaning.

I can say я в/на самом деле хочу спать = I really want to sleep.

Also в самом деле can be used as an agreement with someone in a conversation:
A: - у нас нет хлеба
B to C: - в самом деле, сходи за хлебом -- indeed, go buy some bread

It can also be used for an emphasis:
Да прекратишь ты, в самом деле?! = Stop that, will you?!

Interchangeable? I had learned them to be opposites almost...в is affirmation and на is a reverser I believe. Can someone confirm/deny?

На самом деле is explanatory, - actually, as a matter of fact
в самом деле is affirmative or asking for confirmation - indeed

But in conversational speech в самом деле is sometimes used instead of на... But the reverse is unlikely.

1) Emphasis of the actuality/truthfulness of a
statement/fact:

2) Confirmation of incontestability of a statement:
Александр во время службы в органах не отличался особой
меткостью. Если он на самом деле безупречно владеет оружием,
значит, его научили этому, когда он был «в бегах». (Изв.);

3) Negation of a preceding statement:
Был задержан литовский теплоход
«Расейняй» с 63 тоннами меди и 61 тонной никеля на борту,
предназначенных якобы для Калининградской области,

4) Insistence on one's position:
Сколько раз говорить, что возле этих приборов

5. The two connectors have a substantial commonality,
the confirmation of validity of the message/statement.
The connectors can only be used when the speaker
has no doubts about truthfulness or appropriateness of
what he's saying. In other words, the correct use of these
expressions assumes the truthfulness of the message.
In the simplest case the matter in question
takes place/has taken place. Here one can use either
в самом деле or на самом деле:
Он сделал вид, что спит, и через
полчаса в самом деле (на самом деле) уснул.

When a preceding statement/assumption doesn't take
place in reality, only на самом деле can be used:
– Неужели он болен?
– Нет, на самом деле он здоров.

The connector в самом деле (uses 1 and 2) is often
preceded by the particle и. When в самом деле is used,
the speaker isn't very sure in the discussed statement.
Between the statement and its confirmation there's
a verification procedure. In this case и connects the
statement and its confirmation:
Главный тезис столичного мэра – «оплата жилья должна
соответствовать доходам населения» – будет
принят во внимание. Иначе все и в самом деле закончится
тем, что люди перестанут оплачивать жилье вовсе (Лит.).

6. Expressions в самом деле (use 3) and на самом
деле (use 4) can be used as parenthetical/inserted clauses.
Parenthetical clauses are more emotional.
Here в самом деле is preferred over на самом деле.

На самом деле is often used as an adverb whereas the
use of в самом деле in such a role is considered
substandard, although it does sometimes occur in speech:
Меня считают лучше, чем я есть в самом деле.
We consider this use of в самом деле incorrect.
В самом деле is (incorrectly) used here in place
of на самом деле due to the similarity between the two.

There're also incorrect uses of на самом деле:
Гайдар публично дистанцировался
от Бориса Ельцина – своего «крестного отца». На самом
деле, найдя мужество фактически признать свое
политическое поражение, Гайдар проявил слабость, когда
отказался от дальнейших поисков компромисса (МН).

A statement in which на самом деле is used in this case
confirms or clarifies the validity of
the corollary of the preceding statement. Apparently,
на самом деле replaces here в самом деле.

7. It's also interesting to compare the meanings of:
действительно, в действительности, по существу,
по сути, практически, фактически и т.д.
