How would one translate the following phrases using какого черта?

Why the hell did he do that?
How the hell did he do that?

Would какого черта be used in both questions?

1. Какого чёрта он это сделал?
2. Как он, чёрт возьми, это сделал?

The first I'd say За каким чертом (or на кой черт) он это сделал? The second somehow does not work out with какого черта - I'd say Черт возьми, как ему это удалось?

I'd say "какого чёрта" can mean "why the hell" and also "what on earth": "Какого чёрта ты тут делаешь?!?!?!?!" = What on earth are you doing here?!?!?!?!
- but not "how the hell".

Какм раком, каким макаром он это сделал? (for the second phrase) These are more modern turns of speech - i wouldn't say it's slang, but a sort of youth language.
