If you want to say that you've read a book or watched a movie, do you decline the title? For instance, is it: Сейчас я читаю "Бедных людей" Достоевского or Сейчас я читаю "Бедные люди" Достоевского? For movies, Вчера я посмотрел "Маленькую Веру" or Вчера я посмотрел "Маленькая Вера"?

they say both should be declined: Ja chitaju 'Voinu i mir'.
but alternatively you could say: Ja chitaju knigu 'Voina i Mir' without declining I think.

You do decline them.

However, you don't if there's a classifier noun before the title. So it's «Сейчас я читаю "Бедных людей"» but, «Сейчас я читаю книгу "Бедные люди"», and «Вчера я посмотрел "Маленькую Веру"» but, «Вчера я посмотрел фильм "Маленькая Вера"».

Sure you have to decline it -- if you haven't, there wouldn't be any cases around. ^_^ But be careful: if the title is shielded by any substantivator noun, like "I've seen movie "Кавказская пленница", then that noun should be put into accusative, not the title: "Я читал книгу "Властелин колец".
