Ru-SA-kov or Ru-sa-KOV?

I'd say RusakOv. However RusAkov it's possible too, but IMHO may be very rare.

RusakOv is more common, but RusAkov could be in use too.

I am sure that it should be pronounced "RusaKOv", I've never heard RusAkov and SharapOva.
Only SharApova and RusakOv.

RusakOv if you not know exactly, but this can differ depend on your location. I thisnk RusakOv is more neutral. It is more common.

In case of second name you can be wrong time to time, and owner will fix you. I personally go thru 2 year battle with my Russian literature teacher about my second name forms. Well, i have not native russian second name :)
